A Medicare coverage review checklist you can use every year

A short, organized review can catch a formulary change, network issue, or cost shift before the new plan year begins. It can also confirm that no change is needed.

Gather the current documents

Use the plan's Annual Notice of Change, Evidence of Coverage, and current insurance cards. Bring a current medication list and do not rely on last year's pharmacy printout.

List the providers that matter

Include primary care, specialists, hospitals, and any recurring outpatient facility. Confirm network status for the next plan year with both the plan and provider.

Compare total cost and access

Premium is only one part of the comparison. Review copays, coinsurance, deductibles, annual maximums, drug costs, referrals, prior authorization, and out-of-area needs.

  • Annual prescription estimate
  • Maximum out-of-pocket exposure

Document the decision

Write down why you kept or changed coverage and save the confirmation. If you enroll, review the effective date and watch for the new card. Contact the plan promptly if enrollment information is wrong.
